You become a vampire if you fight a vampire and contract the vampirisim disease (called Polyphoric Hemophilia) and do not take actions to cure it within three days. It is treatable as a common disease. You have a small chance of contracting the disease everytime a vampire hits you, either from punches or a weapon. Spells that are cast upon you by a vampire do not have a chance of transmitting the disease. Note that if you have a resistance or immunity to common diseases (as some races naturally have), it may make it very hard or even impossible for you to contract Polyphoric Hemophilia. In these cases, you may want to find a spell, scroll, or potion that will lower your resistance to common disease.

there is indeed a way to revert back to being 'normal'. If you locate a copy of the book 'Vampires of Vvardenfell Vol. II', it contains information on what to do to rid yourself of the vampirisim curse. Note that once you've reversed the process and return yourself to normal, you will then effectively be immune to their disease, and thus will not be able to become a vampire a second time.

If you're looking for a more specific answer, read on. You need to find the daedric ruins of Bal Ur, located just north of Suran. Inside there you should find a person by the name of Derar Hlervu (spelling may not be exact), and he will instruct you to speak with the statue. You'll be given a quest that involves killing people in a mine called Dubdilla. After that is complete you should be able to return and speak with the statue again to have your curse removed.
